Mentor Application

IMPORTANT: This application requests basic facts to be utilized for the purposes of establishing The Bridge Mentorship Program (the “Program”). Please do not share any sensitive or personal information in this application.

Time Commitment and Expectations

The Program will run for six months. Mentors and mentees are expected to meet a minimum of six times – ideally, once a month. Scheduling will be coordinated by the mentee. In-person meetings are preferred but not required. Mentees are expected to present a specific goal for the engagement and at each session, mentors and mentees should work together to pre-set an agenda for their next meeting. By submitting this application to confirm your agreement to adhere to these participation expectations.
